Home Care Package Team Lead

Glasshouse Country Care has an exciting opportunity for a new position as a Team Lead, leading the organisation’s Home Care Package Team.

The HCP Team Lead will ensure the effective and efficient delivery of the Glasshouse Country Care Home Care Packages. The HCP Team Lead will support a team of Registered Nurses, Care Managers and an Occupational Therapist to coordinate the delivery of services to support frail aged people, who are at risk of requiring premature long-term care, to remain living independently in their own homes.

The HCP Team Lead is responsible for the following:

  • supervision, support and mentoring of staff within the HCP team

  • leadership within the team

  • decision making

  • liaise with senior managers as appropriate, in area of responsibility.

 

The position is responsible for the team’s provision of high-quality client services through assessment, referral, co-ordination, monitoring and case management (including package financial management). This may be achieved by supporting the HCP Team and may include visiting the clients at home, supporting to develop care plans, including budget and implementing service delivery.

Qualifications and Experience

  • Diploma in Community Services or Diploma in Case Management or an equivalent qualification and experience working in the health and/or community care sectors.

  • Demonstrated experience in working with and achieving agreed performance measures including budget expenditure and project outputs.

  • Demonstrated experience in team leadership and management and ability to promote cross-team relationships.

  • Demonstrated experience in working with clinical/allied staff.

  • Understanding of and experience working within the following guidelines/standards:

    • Aged Care Quality Standards

    • Aged Care Act.

Flexible hours Monday – Friday. Just tell us when you’re available.

We ask you to use your own vehicle.

Generous reimbursement based on km travelled.

Full drivers licence required.
