REGISTERED NURSE

Due to an increase of clients, we have an opportunity for another Registered Nurse to join our Home Care Package Team.
​

The Registered Nurse will provide direct care and support to service users of Glasshouse Country Care in their own home. The role takes organisational supervision from the Home Support Coordinator.
Responsibilities:

Providing clinical care for Home Care Package clients.

Assess/reassess/review clients for HCP Level 3 and Level 4 and CHSP.

Administer medications when required including S8 medication.

Cover any nursing issues for HCP, CHSP, NDIS and day centre clients.

Liaise with carers, General Practitioners, Pharmacists and Allied Health.

Ensure the support workers are provided with the clinical support, information and expertise required to effectively meet their responsibilities.

Continence assessments – MASS and CAPS applications.

Provision of quality outcomes for all clients.

Maintain service delivery processes which communicate care, empathy, respect, and dignity for the client.

Promote safety by following organisational WHS requirements and guidelines.

Communicate respectfully with all individuals including service users, representatives, staff and the community.

Provide clinical support to the Continuous Improvement Team.

Contribute to continuous improvement by involvement in staff meetings, planning days, and feedback mechanisms.

Participate in education and training opportunities.

Provide clinical training and support to Personal Care workers.

Ensure that appropriate service user records and data on services is maintained and that regular evaluations of service user need and service delivery occur.
To be successful in this role, you will have:

Registered as a Nurse with Australian Health Practitioner Regulation Agency (AHPRA) and holds a current practising certificate.

A strong commitment to working with older people or people with a disability.

Ability to work in a team and independently.

A motor vehicle driver’s licence and own vehicle.
