To allow us to continue providing you with the highest quality care and support
the price of our Aged Care services will change on 1 August 2022.
All affected clients will receive a letter detailing these changes.

Am I eligible?
Eligibility is based on need. You may be eligible for services if you have:

noticed a change in what you can do or remember
-
been diagnosed with a medical condition or reduced mobility
-
experienced a change in family care arrangements
-
experienced a recent fall or hospital admission.
You must also be:
65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
50 years or older (45 years or older for Aboriginal and Torres Strait Islander people)
and on a low income, homeless, or at risk of being homeless.

My Aged Care
Before you can access government-subsidised aged care services for the first time, you need to apply for an assessment.
Check your eligibility and apply for an assessment online or call My Aged Care
on 1800 200 422.
Step 3
Ask for Glasshouse
Country Care to be your service provider
My Aged Care will give you a referral code. Once we have this code Glasshouse Country Care are able to provide you with services.

You can change providers at any time by contacting My Aged Care.
Step 4
Establish your
Care Plan
Our staff will arrange a time with you to discuss the range of services available to meet your needs.
Together we will decide
on a care plan, when
services will commence
and the cost to you.
