
Support at Home
Support at Home is a new program designed to make it easier for older Australians to get the help they need to live independently. It brings together existing in-home aged care services into a simpler, fairer system.
Whether it’s cleaning, personal care, meals, or getting out and about, Support at Home offers more flexible, tailored support helping you
to stay healthy, active, and connected to the community.
The Support at Home program replaced the Home Care Packages (HCP) Program on 1 July 2025. The Commonwealth Home Support Programme (CHSP) will continue operating as usual before becoming part of the Support at Home program no earlier than 1 July 2027.

How to access Support at Home?
If you are new to Aged Care
The first step is to contact My Aged Care to check your eligibility and apply for an in-home assessment.

If you already have a Home Care Package
If you already have a Home Care Package, or have been approved for a Home Care Package, you will automatically be transitioned to Support at Home.
You do not need to be reassessed, and your services will continue.
We will work with you to manage the transition.
Your funding will be the same as your current HCP level and any unspent funds will carry over for you to spend on services, assisted technology or home modifications.
