Home Care Package Care Manager (Registered Nurse)

Glasshouse Country Care provides support services to elderly and disabled clients to help them live safely and comfortably in their own homes and participate in their community.  Glasshouse Country Care has been providing support services in our local community for over 23 years.  We are an approved provider for the Commonwealth Home Support Service, Home Care Packages and NDIS.

 

We are looking for a Care Manager/Registered Nurse who will provide clinical care, case management and care coordination for home care package clients using person centred and consumer directed care models for service delivery. 

Responsibilities:

  • Supporting the Clinical Supervisor with Clinical Care for Home Care Package clients

  • Supporting the Clinical Supervisor to assess/reassess/review clients for HCP level 3 and 4

  • Accept HCP referrals via MAC (My Aged Care) Portal and enter relevant data into the data management system

  • Visit new HCP clients in their home environment for signing of their contract

  • Development of a care plan in conjunction with client and his or her advocate

  • Individual service that meets the client’s individual needs

  • Regular monitoring, maintenance and review of clients' care needs and care plan

  • Approving of services in the relevant database

  • Productive liaison, referral, and negotiation with other community services, especially ACATs (Aged Care and Assessment Teams), GPs, Social Workers, ethnos specific services and other relevant service providers

  • Liaison with Rostering Officer for in home care package clients and HCP Admin Assistant on clients' behalf

  • Manage client budgets.

 

To be successful in this role, you will have:

  • Registered as a Nurse with Australian Health Practitioner Regulation Agency (AHPRA) and holds a current practising certificate (For nursing discipline)

  • Demonstrated case management experience and/or knowledge of the Community/Aged Care system and home care packages

  • Exceptional customer service, negotiation and influencing skills

  • Experience in a result driven environment

  • Excellent ability to connect with multiple stakeholders and think commercially

  • Experience using a customer database

  • Experience in working with older people or those with disability

  • Experience with working with clients' families

  • Relevant experience in Community Care

  • First Aid Certificate (or willingness to obtain one)

  • Yellow card (or willingness to obtain one)

  • Current Driver Licence.
