CARE PARTNER (SUPPORT AT HOME)

 

Join our team and make a meaningful difference in the lives of older people. As a Care Partner, you’ll play a vital role in supporting clients to remain safe, independent, and connected at home. You’ll work alongside each person to understand what matters most to them and help turn their goals into reality—guided by reablement principles, dignity, and the Aged Care Act 2025.

 

Key Responsibilities

  • Assessment & Care Planning: Conduct comprehensive intake assessments; develop and review Goal-Directed Care and Reablement Plans using a rights-based, least-restrictive approach.

  • Care Coordination: Coordinate services to support client goals; maintain regular monitoring; manage changes in needs or risk; liaise with families, GPs, allied health, hospitals, and other providers.

  • Safety & Legislative Compliance: Ensure practice complies with the Aged Care Act 2025, Strengthened Quality Standards, and Code of Conduct. Manage risk, incidents, SIRS reporting, and uphold privacy, dignity, and boundaries.

  • Associated Provider Coordination: Arrange referrals; review provider suitability and reports; ensure compliance and escalate concerns.

  • Financial Responsibilities and Program Compliance: Support clients to understand budgets and service inclusions; monitor spending and pricing compliance; ensure accurate service recording and identify funding risks.

  • Professional Conduct & Improvement: Maintain ethical practice; participate in audits, meetings, training, and quality initiatives; uphold cultural safety and trauma-informed practice.

  • Organisational Responsibilities: Liaise on rostering needs, advocate for consumers, comply with WHS, communicate respectfully, and contribute to continuous improvement and professional development.

 

What you will bring:

  • Certificate IV in Aged Care (or equivalent).

  • Experience in case management, community care, or home care programs.

  • Warm, professional communication skills and a genuine commitment to exceptional customer service.

  • Strong negotiation, relationship-building, and problem-solving abilities.

  • Ability to work confidently in a dynamic, outcomes-focused environment.

  • Experience using client management systems or customer databases.
