Create your Home Care Package with Glasshouse Country Care to best support your needs and goals by working in partnership with us. Being partners is an important part of the process, and we will help you to get the most out of the care and services available to you.

The range and level of services you receive under your home care package depends on your assessed care needs and the funds available under your Home Care Package. The Aged Care Assessment Team (ACAT) will do this assessment and choose which level of package is suitable for you.
THERE ARE FOUR LEVELS OF HOME CARE PACKAGES:
HOME CARE LEVEL 1: SUPPORTS PEOPLE WITH BASIC CARE NEEDS.
HOME CARE LEVEL 2: SUPPORTS PEOPLE WITH LOW LEVEL CARE NEEDS.
HOME CARE LEVEL 3: SUPPORTS PEOPLE WITH INTERMEDIATE CARE NEEDS.
HOME CARE LEVEL 4: SUPPORTS PEOPLE WITH HIGH CARE NEEDS.

This is the best part! You tell us what you need and we make it happen, whether that’s getting help with showering or house cleaning, getting your hair done, or organising your transport for social support or shopping.
As part of developing and reviewing your care plan we will work with you to develop a budget for your Home Care Package (HCP). Your proposed budget will outline your HCP income and expenditure and be easy to understand.
Every Home Care Package is different. Our qualified team discuss with you and your nominated representatives your individual needs and goals. We can tailor our fees to suit you. Our administration and case management costs equate to 15% each of the Level 1 & Level 2 package value and for Levels 3 & 4 Administration is 15% and case management is 20%. The only standard fee in our Home Care Packages is our exit fee of $250.

By accessing social services support we can help you maintain an active social life in the local community which is important for your wellbeing.
We have experienced property maintenance workers ready to help you maintain your home. From lawn mowing to changing lightbulbs.
We operate a pre-booked community transport service weekdays allowing our clients to access appointments, shopping trips and local activities.
We provide delicious and nutritious home cooked meals delivered to your door via a meals on wheels style service 3 days a week.
Day Activity Program
Our group social support program is designed to enrich your days and stimulate your mind mixing with people of common interests.
We have qualified staff that can change that step into a ramp to make moving around your own home easier or add a grab rail to the shower.
