How to access services
MY AGED CARE
Before you can access government-subsidised aged care services for the first time, you need to apply for an assessment.
Check your eligibility and apply for an assessment online, or call My Aged Care on 1800 200 422.
It’s the same form to apply for all types of care and support and you can complete the application yourself or on behalf of a family member or friend.
ASK FOR GLASSHOUSE COUNTRY CARE TO BE YOUR SERVICE PROVIDER
My Aged Care will give you a referral code. Once we have this code Glasshouse Country Care are able to provide you with services.

You can change providers at any time by contacting My Aged Care.
Our staff will arrange a time with you to discuss the range of services available to meet your needs.
Together we will decide on a care plan, when services will commence and the cost to you.
You will need to sign a Home Care Agreement with us.
