Home Care Packages
You may find that some things are becoming more difficult for you to manage and in order to stay at home you need extra help. A Home Care Package may assist you to do this in conjunction with your family, friends and other community resources.
You can tailor your Home Care Package with Glasshouse Country Care to best support your needs and goals by working in partnership with us. Being partners is an important part of the process, and will help you to get the most out of the care and services available to you.

Am I eligible?
Eligibility for a Home Care Package is determined by an Aged Care Assessment. The Aged Care Assessment Team (ACAT) will do the assessment and choose which level of package is suitable for you and what range and level of services you will receive.
Each package level has a different amount of funding (subsidy) that is paid to your provider by the Australian Government to deliver your care and services. You can find out more on the subsidy rates at the Department of Social Services' website at

What will I need to contribute?
The total amount of a Home Care Package is made up of what the Australian Government pays (the subsidy) and what you may need to pay (your home care fees).
The subsidy the Australian Government pays will be different for each level of Home Care Package. This amount is paid to the home care provider you choose.
Home care packages are income tested. You are expected to contribute to the cost of your care and services if you can afford it.

If you are self-funded or do not receive a pension, you will need to complete an Aged Care Fees Income Assessment Form.

Depending on the level of Home Care Package you receive, you can get assistance with a range of different services including:
Showering and grooming, dressing, mobility in your home, help with medication.
Help in your home
Cleaning, house and yard maintenance, washing and ironing assistance, meal preparation and delivery, shopping services.
Our transport team provides a door-to-door service to take you to health appointments, social activities and shopping.
Social Activities and Support
We offer a variety of community activities daily so you can continue socialising and building friendships in the community.
Nursing services, referrals to Allied Health and other clinical services to help you maintain movement and mobility.
